How Many Drops of Methylene Blue Should I Take? What Actually Determines the Answer

No drop count for methylene blue has been established by any regulator, and none appears on any FDA-approved label. The only dose printed on an approved methylene blue label is 1 mg per kilogram of body weight, given intravenously over 5 to 30 minutes, to treat acquired methemoglobinemia. That is the prescribing information for ProvayBlue (methylene blue injection, USP, 5 mg/mL, revised February 2024), a prescription-only product. Nothing approved is swallowed, and nothing approved comes with a dropper. If your bottle says 1%, it holds 10 mg per mL, and a dropper built to the USP standard of 20 drops per mL delivers about 0.5 mg per drop, which makes two drops roughly 1 mg. That arithmetic describes what is in the drop. Whether any amount belongs in you is a determination that requires your full medication list and your G6PD status, and it is made by a prescriber.

The only dose that carries label authority

ProvayBlue's dosing section runs five sentences, and every number in it is attached to a hospital. Administer 1 mg/kg intravenously over 5 to 30 minutes. If the methemoglobin level remains above 30% or clinical signs persist, a repeat dose of 1 mg/kg may be given one hour after the first. If methemoglobinemia does not resolve after two doses, the label instructs the clinician to consider alternative interventions. Two doses, one hour apart, then stop and try something else.

The same label records what sits above that line. A cumulative dose of 7 mg/kg or more of a methylene blue product caused nausea, vomiting and precordial pain. A single dose of 20 mg/kg or more caused severe intravascular hemolysis, hyperbilirubinemia and death.

Run that dose through a dropper and the mismatch is immediate. A 70-kg adult's 1 mg/kg dose is 70 mg. From a 1% oral solution at 10 mg/mL, that is 7 mL, or about 140 standard drops. Nobody swallows 140 drops. The regimens circulating online, usually 2 to 10 drops of a 1% solution, deliver 1 to 5 mg, somewhere between 1.4% and 7% of the only weight-based dose that appears on any label for any indication. The label number does not scale down to a dropper. It was written for converting methemoglobin back into hemoglobin in a patient whose blood has stopped carrying oxygen.

Work out what one drop of your own bottle contains

  1. Read the concentration printed on the label. It will be a percentage or a mg/mL figure. If it is neither, this calculation cannot be done.
  2. Convert it. A 1% aqueous solution is 10 mg per mL. A 0.5% solution is 5 mg per mL, the same strength as the FDA-approved injection.
  3. Divide by the number of drops your dropper delivers per mL. The USP default is 20.
  4. Ask the seller, in writing, for the certificate of analysis with a lot number matching your bottle.

Step four is the one people skip. I spent my career requesting medical records for a state disability determination service, where verification is documentary and unimpressed by volume. One clinic answered every request for a single discharge summary with four hundred pages, and the summary was often not among them. A seller who sends a certificate of analysis with a matching lot number has answered the request. A seller who sends you a page about mitochondria has sent four hundred pages.

A drop is not a unit of measurement

USP General Chapter 1101, the medicine dropper monograph, defines the pharmacopeial dropper as a tube constricted at the delivery end to a round opening about 3 mm in external diameter which, held vertically, delivers water in drops weighing between 45 and 55 mg each. At water's density, the 50 mg midpoint is 0.05 mL, and that is the entire origin of "20 drops per mL". The chapter then adds the caveat almost nobody repeats: most medicinal liquids do not share water's surface tension and viscosity, so drop size varies from one preparation to another, and even with a dropper calibrated for its own liquid, the volume error should not exceed 15% under normal use.

Fifteen percent is the standard's tolerance. Real bottles do worse. Moore, Beck and Kryscio tested 192 bottles from 32 bottle designs and manufacturers for BMC Ophthalmology in 2017 and found delivered drops per mL ranging from 20.9 to 40.8. At 40.8 drops per mL, ten drops is 0.245 mL. At 20.9, the same ten drops is 0.478 mL. Identical count, nearly double the dose. Twenty-two of the 32 designs also gave a significantly different mean number of drops depending on whether the bottle was held vertical or horizontal, so the angle of your wrist is a variable too.

Those were ophthalmic squeeze bottles rather than the glass pipettes sold with methylene blue, and I found no published measurement of supplement dropper bulbs at all. I once marked a file complete when a records request had bounced, and a claimant waited four months for a determination that could not be made, because the record was short one document and nothing on its face said so. Your bottle's true drop volume is that document. The arithmetic circulating online proceeds as though it were already in the file.

Where the comparison with liquid supplements breaks

Anyone counting methylene blue drops is borrowing arithmetic from vitamin D drops, and the borrowing holds right up to the point where it matters. Vitamin D has a Recommended Dietary Allowance of 600 IU a day for adults through age 70 and a Tolerable Upper Intake Level of 4,000 IU for ages 9 and older, set by the Food and Nutrition Board in 2011. The ceiling was derived by taking roughly 10,000 IU, below which hypercalcemia is unlikely, and dividing by an uncertainty factor of 2.5. A drop 40% larger than intended lands well inside that margin, and nothing happens.

Methylene blue has no Recommended Dietary Allowance, no Upper Intake Level and no uncertainty factor, because nobody has calculated one. FDA's Dietary Supplement Ingredient Directory, current as of 1 May 2026, carries no entry for methylene blue at all. The two numbers that exist are a labelled therapeutic dose of 1 mg/kg and an observed-harm threshold of 7 mg/kg cumulative, and the second one describes what happened to people rather than a ceiling anyone calculated. Between them sits a factor of seven, measured in monitored patients by the intravenous route.

Two questions that come before the arithmetic

The first is whether you have G6PD deficiency, which is a flat contraindication on the ProvayBlue label, because methylene blue can trigger hemolytic anemia in people who carry it. It is identified by a blood test that most adults have never had.

The second is the boxed warning. Methylene blue may cause serious or fatal serotonin syndrome in combination with serotonergic drugs and opioids, and the label directs prescribers to avoid concomitant use with SSRIs, SNRIs, MAOIs and opioids. The interaction section widens the list to bupropion, buspirone, clomipramine, mirtazapine, linezolid and dextromethorphan, which is the active ingredient in ordinary cough syrup. Where intravenous use cannot be avoided in someone on serotonergic medicines, the label says to choose the lowest possible dose, observe for CNS effects for up to four hours, and advise the patient against serotonergic drugs for 72 hours after the last dose.

The reported cases behind that warning involved 1 to 8 mg/kg intravenously, most of them during parathyroid surgery, in patients taking an SSRI, an SNRI or clomipramine. Whether the same risk applies by mouth, or at doses far below 1 mg/kg, is described in the professional monographs as unclear. Unclear is a finding. It means the evidence was requested and did not arrive, and a file in that condition supports no determination in either direction.

What "how fast does it work" is actually measuring

The approved product's clock measures a laboratory value. Infuse over 5 to 30 minutes, remeasure methemoglobin, decide at one hour whether to give a second dose, and after two doses move to something else. Nothing in that sequence tracks how anyone feels.

By mouth, the variability is larger than people assume. Walter-Sack and colleagues gave 16 healthy volunteers 500 mg of an aqueous oral solution and 50 mg intravenously a week apart, and reported absolute oral bioavailability of 72.3% with a standard deviation of 23.9 percentage points, in the European Journal of Clinical Pharmacology in 2009. The spread is nearly a third of the mean. Two people counting the same drops from the same bottle can absorb substantially different amounts. An earlier study in the same journal, in 2000, estimated a terminal half-life of about 5.25 hours after intravenous dosing.

Onset figures on vendor pages and forums are reported by feel, usually inside half an hour. I started writing this before it was light, because the mornings keep arriving earlier now and you can tell, and anything taken at that hour feels like it worked. Blue-green urine arrives quickly and records that the dye is being cleared. Methylene blue in the blood also makes a pulse oximeter underestimate oxygen saturation, which is on the label.

Four documents decide this question: the concentration on your bottle, the volume your dropper actually delivers, your current medication list, and your G6PD status. Most people asking about drops are holding one of the four. The determination was never mine to make, and it isn't the bottle's either.
